We use this information to respond to your inquiry and consider practical coordination options for a planned healthcare visit.
Do not submit passport numbers, payment information, detailed medical records, imaging files, or other highly sensitive personal information through the website form or ordinary email. If additional documents are needed after an initial response, we will discuss an appropriate way to share them.
The website enquiry form uses FormSubmit to deliver your message to IntersectionMed at intersectionmed@gmail.com. Submitting the form sends the information you enter through that service. We recommend sharing only the minimum information needed for an initial, non-emergency inquiry.
